United Jewish Council of the East Side (UJCES), a CBO on the Lower East side of Manhattan is looking for a dynamic person to join our team of professionals as a Case Worker for our senior center.

Education: Minimum BA/BSW required and supervised casework experience with seniors preferred.

Preferred Experience:
• Knowledge of types of entitlement/benefits for older people, and eligibility and application requirements.
• Knowledge of local and citywide resources for older persons.
• Ability to accurately prepare entitlement applications and to assist clients in completing applications.
• Interviewing, assessment, and case note writing skills.
• Ability to make appropriate referrals and to follow-up in a timely manner.
• Ability to work with clients empathetically and respectfully.

Responsibilities:
To provide case assistance and case management (according to DFTA guidelines) to seniors.
• Provide home visits to the frail elderly for the purpose of assessing client’s eligibility for the UJC homebound lunch program.
• Provide home visits to do reassessments on a six-month basis of clients already on the lunch program.
• Provide home visits to the frail elderly who need assistance with benefits and entitlements, financial arrears, with assessing client’s needs for homecare and other supportive services that will enable them to remain safe in the community.
• Provide information and referral to walk-in clients or to those who call for assistance.
• Assist clients who can come to the office with a wide array of services. These services include providing entitlement and benefit counseling and assessing the client’s eligibility for benefits and entitlements, completing applications for SSA, SSI, SSD, public assistance, food stamps, etc.
• Register clients in PDS and A2B programs.
• Complete monthly statistical reports documenting case assistance and information and referral.
• Accompany clients to city benefit offices such as public assistance, social security, Medicaid and HRA to advocate on their behalf at fair hearings and for application process.
• Pre-screen clients for Medicaid.

The Social Worker will report to the Director of the Senior Center

Salary: $50,000 a year plus fringes.
